# Update Personal Information Form

## Goldman Sachs

- Name changes require supporting documentation (marriage certificate, divorce decree, or other legal documents).
- Physical address may not be a PO Box.
- If your client is adding their email address for the first time, the delivery of reports and statements will be updated to email rather than mail.
- Requires client signature(s). LPOA does not apply.
